Long Term Hospitalization
Three (3) or more cumulative years in an inpatient psychiatric hospital, within the last five (5) years.
OR
Jail Diversion
Two (2) psychiatric crises (i.e., inpatient psychiatric hospitalizations and/or outpatient psychiatric crisis) and four (4) arrests within the last three (3) years.
Must have active Medicaid benefits
OR
Emergency Department (ED)
Two (2) or more psychiatric crises (i.e., inpatient psychiatric hospitalizations and/or outpatient psychiatric crisis) and fifteen (15) ED visits within the last three (3) years.
Must have active Medicaid benefits
